Orientation and Mobility Consultative and Teaching Services (Request for Proposal)

Description: Provide the board with current documentation verifying provider is certified in orientation and mobility and/or visual impairment and qualified to conduct evaluations for visual impairment and orientation and mobility. Complete the appropriate evaluations and individual education plan (iep) services as indicated on the evaluation referral form and iep required by the west virginia department of education state board policy 2419: the regulations for the education of exception students. Submit logs of services for each student served. Provide evaluations as determined by the sat or iep team.
