Provide up to twelve (12) months of comprehensive care coordination, service navigation, peer support, and housing-related assistance to persons who are experiencing homelessness (PEH), or are at risk of homelessness, who are justice- involved, and who have a history of mental illness or substance use disorder, with additional complex factors such as physical health, social, income and legal needs. This program will leverage Medi-Cal and triage individuals to the appropriate level of care. This program is designed to improve community safety through services to reduce justice system involvement for these individuals while also supporting improved health outcomes.
