Provider-Facing Population Health, Care Coordination, and Interoperability Solutions (Request For Information)

Description: North Dakota Health and Human Services has issued a Request for Information to collect information about solution options and functionality of population health data systems for use by rural healthcare providers across the state. The RFI seeks vendor input on provider-facing population health, care coordination, and interoperability solutions that can integrate with electronic medical records, enable real-time clinical interventions, and connect rural hospitals, clinics, specialty providers, behavioral health practices, pharmacies, and community-based organizations. Responses addressing data integration, care coordination, referral management, patient engagement, and value-based care analytics are due by September 2, 2026, with questions accepted through July 29, 2026.
