Hospital rate setting services. The core purpose of the Wisconsin Medicaid program is to make appropriate medical care available to people in specific age, family status, or disability groups who cannot afford necessary medical care. Medicaid eligibility includes elderly, blind and disabled individuals. Contract and shall run for two (2) years from that date, with an option by mutual agreement of the agency and the supplier, to renew for an additional three (3) one (1) year terms. Provider to design and implement a pilot program and provide in-home health care and support services for elderly individuals who do not qualify for existing programs, such as medicaid and cannot afford in-home health care or support services on the island..
