Required elements are: 1. Collaboration with community partners which assist in identifying and accessing the target population. Suggested partners include the Department of Social Services, Health Department, Local Behavioral Health Authority, Area hospitals, Judy Center, Birth-to-five, Infants and Toddlers programming.
