Description: Qualified vendors to contract for FY27 - family re-entry. Applicants will propose a program/model design that addresses the needs of the target population. Required elements are: 1. Provide peer support services along with care coordination and transportation 2. Provide assistance with social services such as applications for SNAP, Medicaid, MDRN provided assistance, disability, ID/birth certificates, housing, and employment. As well as connecting individuals with treatment services and health care. See attached file.

Seeking proposals from qualified Vendors to contract for FY27 - Early Childhood ACEs. Applicants will propose a program/model design that addresses the needs of the target population. Required elements are: 1. Collaboration with community partners which assist in identifying and accessing the target population. Suggested partners include the Department of Social Services, Health Department, Local Behavioral Health Authority, Area hospitals, Judy Center, Birth-to-five, Infants and Toddlers programming. Once awarded, the selected provider will need to demonstrate formal partnerships through MOU, contracts, or other written agreements. See attached file.
