FY27 - Navigation Services (Request for Proposal)

Description: Seeking proposals from qualified vendors to contract for FY27 - navigation services. Applicants will propose a program/model design that addresses the needs of the target population. Required elements are: 1. Demonstrate a strong understanding of local resources including an established network allowing for incoming and outgoing referrals 2. Must utilize promising practices, best practices, and/or evidence-based practices demonstrating that the strategy has verifiable outcomes. 3. Incorporation of intentional efforts to address racial and ethnic disparities. See attached file.
