This document outlines the Department of Disabilities, Aging and Independent Living's (DAIL) improvement and action steps to strengthen Vermont's Choices for Care home and community-based services system. It addresses areas of weakness identified in the State's Comprehensive Quality Strategy, specifically regarding person-centered planning and home- and community-based settings standards. The plan focuses on regulatory compliance and enhanced person-centered planning for Home-Based Settings (Adult Family Care and Adult Day) and Non-home-based Enhanced Residential Care settings.

The area plan for Age Well focuses on supporting diverse older Vermonters, coordinating services to help them live in their preferred settings, and improving the quality of life through various programs. Key areas include community planning, case management, nutrition, health promotion, caregiver support, and prevention of elder abuse. The plan aims to increase access to services, enhance community partnerships, and ensure the well-being of older adults through coordinated and client-centered approaches.

The Area Agency on Aging (AAA) Area Plan Instructions for FFY 2022-2025 outlines the requirements and guidelines for developing and submitting area plans. It covers various sections, including verification of intent, needs assessment, focal points, goals, data management, continuous quality improvement, and public hearings. The plan emphasizes compliance with the Older Americans Act, coordination of services, and prioritization of those in greatest economic and social need. It also includes budget information, reporting timelines, and required assurances.

The meeting included discussions and decisions on several key topics. The commission approved the September minutes and reviewed the 2023 Annual Report, assigning reviewers for the finalized draft. Legislative priorities for 2024 were discussed, including increasing the Dementia Respite Grant, enhancing residential care through the Match Study, restoring funding for the Moderate Needs group, incorporating ADRD into brain health campaigns, and advancing housing access and security for individuals with ADRD. Updates were provided on educational resources for hospital staff and the alignment of the report with DLP proposed RCH and ALR. The DEI Workgroup is set to research findings to identify gaps and determine a new starting point. Additionally, there was a BOLD VDH update, including plans for a 5-year plan and partnering with Blueprint and Alz Association for community health team education. The DAIL update covered the new Specialized/complex care facility ICARE Health Network and the State Dementia Coordinator.

The meeting included a presentation from the Health Care Advocate & Ombudsman regarding support for Vermont residents' access to care. An update on DAIL Long Term Care Regulation revisions was provided, including ADRD additions related to safety, quality of care, and training. There was also a report on Professional Educational Opportunities, with discussion on additional research needs. DAIL announced a new community grant opportunity for services, staff recruitment, and capital improvements. Updates were given on BOLD and BRFSS data, and the progress of Dementia Friendly Community initiatives.
