Family Caregiver Counseling Services (Request for Proposal)

Description: a. Provide group and or individual counseling and training by systematically seeking out caregivers in key geographic areas, such as senior centers, senior clubs, nutrition sites, or in the community abroad. b. Screen and assess the caregivers for caregiver burnout and the care recipients' situation. c. Provide individual and group guidance to enable caregivers to resolve problems and relieve stress. d. Coordinate or provide training to caregivers and family caregivers that focus on issues of interest to caregivers or to care for a specific care recipient. Training may include but not limited to care issues around ADL limitations, the caregiver's role, disease prevention and remediation, available resources, legal options, stages of diseases, financial issues, long range planning, or in-home health and safety issues.
