Housing Support Program (HSP) (Request for Pre-Qualification)

Description: Provide clients as long as needed by providing care and supervision in a home environment, linking clients with needed health care services, and helping clients participate and connect with community-based activities and social networks. The hsp contractors shall provide a program of care to accomplish the following goals: reduce severity of mental health symptoms; improve daily functioning; improve overall health status; increase community connections/social networks; obtain needed mental health, substance use, and primary care services.
