Description: The City and County of San Francisco, through the Office of the Sheriff, seeks a nonprofit contractor to provide housing and navigation services to people transitioning from incarceration through the HEARTSS program. Services include housing navigation, case management, peer support, referrals to housing and treatment, temporary bridge housing, and flexible funds for individual barriers to reentry. The program particularly serves Latine and Spanish-monolingual adults with substance-use and/or mental-health needs, with a stated contract period from January 1, 2026, through June 30, 2028.
