Shared-Housing Model Program - CD3 (RFP)

Description: Provider will deliver intensive case management services with peer-supported case management services. Select service provider will establish meaningful relationships with shared- housing participants and connect participants with referrals to health, behavioral health, substance use disorder, emotional support and group meetings services. Select service provider will also provide augmented supportive services, such as employment coaching, connections to jobs, job training programs and workforce development programs to participants to help build household income in order to take full responsibility for rents by the time of program completion.
