Description: Seeking from qualified governmental and non-profit entities to provide services to persons with HIV. Services to be contracted include Core Medical Services: Early Intervention Services, Health Insurance Premium and Cost-Sharing Assistance for Low-Income Individuals, Medical Case Management (including Treatment Adherence Services), Mental Health Services, Oral Health Care, Outpatient/Ambulatory Health Services (including Laboratory Diagnostic Testing and Specialty Outpatient Medical Care), Substance Abuse Outpatient Care- MAI only and Support Services: Emergency Financial Assistance, Food Bank/Home Delivered Meals, Health Education/Risk Reduction- MAI only, Medical Transportation, Non- Medical Case Management Services, Other Professional Services-Legal Services, Outreach Services- MAI only, Referral for Health Care and Support Services- MAI only.
