Client appeals (re: refusal of prescription of controlled substances; appeal of service ineligibility). Types of data that would need to be collected and stored include: Reporter information, identification of staff and others involved, reporter’s supervisor. Client contact information. Access granted to staff and providers, including: ALL organization employees (clinical and non-clinical). External providers with whom the organization contracts to provide services or client care.
