Early Intervention for the Deaf/Hard of Hearing RFP

Description: Ohio Deaf & Blind Education Services is seeking an independent contractor to provide birth-to-three early-intervention hearing services for children who are deaf or hard of hearing. The contractor will conduct functional evaluations, participate in Individual Family Service Plan development, provide direct hearing services, consult with the core early-intervention team, and attend monthly meetings and program-approved trainings. The RFQ requests a technical approach and work plan, personnel qualifications, a cost proposal, references, and required credentials.
