The contractor will conduct functional evaluations, participate in Individual Family Service Plan development, provide direct hearing services, consult with the core early-intervention team, and attend monthly meetings and program-approved trainings. The RFQ requests a technical approach and work plan, personnel qualifications, a cost proposal, references, and required credentials.
