Qualified community mental health providers to render targeted case management (TCM) for adults within the fee-for-service (FFS) public behavioral health system (PBHS). Services shall be provided to: Adults who have a serious mental health disorder, diagnosed according to a current diagnostic and statistical manual of the American Psychiatric Association that is recognized by the Secretary, and who are (DSM): at risk of, or need continued community treatment to prevent inpatient psychiatric treatment; Older individuals, age 65 or older, or younger adults ages 18 through 21 who have been discharged from inpatient treatment in an Institution for Mental Disease (IMD); at risk of or need continued community treatment to prevent being homeless; or at risk of incarceration or recently released from a detention center or prison.

Provide mental health tele-therapy services through a referral management system of care. The service will provide equitable access to mental health services that include short-term tele-therapy direct services, through a centralized system for all students, at no cost to the student or their family.

Hhsc is seeking applications to establish contract(s) for the provision of home and community-based services - adult mental health (hcbs-amh) for adults with a diagnosis of serious mental illness and have a history of at least one of the following additional criteria: long-term psychiatric hospitalization(s); frequent arrests and psychiatric crisis; and frequent emergency department (ed) visits and psychiatric crisis.
