This document is a Community Health Needs Assessment (CHNA) for the Northeast Georgia region, conducted collaboratively by six hospitals with Northeast Georgia Health System as the coordinating partner. The assessment, a triennial process, aims to identify and prioritize unmet community health needs by analyzing data, gathering community input, and evaluating past efforts. The key strategic priorities identified for Fiscal Year 2022 are Mental and Behavioral Health, Access to Care, and Healthy Behaviors, with an overarching focus on achieving greater health equity by addressing socioeconomic barriers.
