The contractor shall provide all personnel, equipment, supplies, facilities, buildings, transportation, tools, materials, supervision, and other items, and non-personal services, necessary to perform occupational health services, utilizing the contractors private provide network and company owned company operated client portal system, as defined in this performance work statement (pws) except for those items specified as government furnished property and services. The contractor shall perform to the standards in this contract. The contractor will complete services for foh in accordance with work orders and referral documents. Foh will send appointment referrals to the contractor to schedule clinical services with health care providers. Once examinations are complete, the contractor will return the results of the examination to foh. When consultation is requested, the contractor will return the results of the consultation which may include medical evaluation results, responses to medical questions, and the consultant’s medical recommendations, as requested, to foh. The work performed under this contract requires the contractor to: accept service referrals (via work orders and referral documents), assign provider(s) who possess the appropriate credentials and expertise to provide the requested service, schedule patients and confirm appointments.
