UC Davis Medical Center Implementation Strategy 2025-2027
This document outlines UC Davis Medical Center's Implementation Strategy for the 2025 Community Health Needs Assessment, designed to guide resource investment and reduce health inequities in Sacramento County. The strategy identifies and addresses eight significant health needs: access to basic needs such as housing, jobs, and food; access to mental, behavioral, and substance use services; access to quality primary health care services; health equity; system navigation; safe and violence-free environments; access to specialty and extended care; and injury and disease prevention and management. The plan aims to improve community health outcomes by focusing on both medical needs and the social drivers of health, with a specific emphasis on vulnerable populations and communities experiencing health disparities.
