Crawford County Health & Human Services and Crossing Rivers Health Community Health Improvement Plan 2025-2027
The Community Health Improvement Plan (CHIP) addresses health needs identified in the Community Health Needs Assessment (CHNA) to improve the health and well-being of individuals in Crawford County and the Crossing Rivers Health service area.
