Iowa Health and Human Services Public Health System Alignment Update 2027-2028
This document outlines Iowa Health and Human Services' strategy for public health system alignment, aiming to create a more coordinated and efficient statewide service system that better meets the needs of Iowans. The strategic approach involves establishing clear system outcomes, creating consistent pathways for service access, using existing funding more effectively, and streamlining administrative processes. The plan is structured around a Shared Responsibility Model, delineating roles for Iowa HHS, Lead Entities, and local providers. Phase 1, from January 2027 to June 2028, focuses on district public health service system analysis and plan development. Phase 2 and beyond, commencing July 2028, involves implementing these district plans, ensuring the delivery of five core public health services, and redirecting funding streams to align with identified priorities.
