State Monitoring Services - (Request for Proposal)

Description: Monitoring services and intends to use the results of this process to award multiple contracts. Monitoring services are provided by third-party independent Contractors that will be responsible for observing and assessing the operations of Wisconsin SNFs that have demonstrated poor performance, thereby placing residents at risk of serious harm. The contract shall be effective on the date indicated on the purchase order or the execution date of the accompanying contract and shall run for one (1) year from that date, with an option by mutual agreement of the agency and the vendor, to renew for an additional four (4) one (1) year terms.
