Provide Evidence Based Programs - (Request for Proposal) -*REBID*
Evidence based programs include, but are not limited to: matter of balance, chronic disease self-management, and diabetes self-management. Proposals may be submitted for one or more services to begin July 1, 2027 and concluding June 30, 2028. A one-year contract will be awarded, beginning July 1, 2027 and ending June 30, 2028 containing a contingency clause permitting renewal for each year of the remaining contract based upon satisfactory performance.
