OCCUPATIONAL MEDICAL SERVICES AND REVIEW

Description: Thank you for considering our Request for Proposal (RFP) to provide occupational medical and review services for the City of Fountain Valley (hereinafter referred to as “the City”).  Please follow the instructions below. The City is seeking proposals from qualified medical facilities/physicians to provide medical screening services including the following: Medical evaluation services in connection with pre-employment examinations, evaluation and treatment of workers’ compensation injuries and illnesses and other evaluations as required. All services performed must comply with applicable regulations including but not limited to the Division of Workers’ Compensation, US Department of Transportation, CA Department of Motor Vehicles, Cal OSHA, California Commission on Police Officer Standards and American with Disabilities Act. Submitted proposals must meet all requirements set forth in this Request for Proposal.  Proposed fees should be effective as of July 1, 2027, and effective for the entire term of the agreement.  This is not in any way to be construed as an agreement, obligation, or other contract between the City and any person or firm submitting a proposal.  Proposals submitted in response to this request become the property of the City and are subject to the provisions of the California Public Records Act.  This act is designed to give reasonable public access to information in the possession of public agencies.  Bidders must clearly and judiciously designate proprietary information or materials, which information and materials will be returned if requested.   Proposals, prepared according to the following detailed instructions, must be received via PlanetBids no later than 4:00 p.m., Pacific Standard Time, Tuesday, December 8, 2026.
