Description: Amendment 1 revises Attachment D – Price Schedule to provide separate pricing elements for bulk liquid medical-grade oxygen, routine oxygen delivery, H-Size reserve oxygen cylinder rental, oxygen cylinders, cylinder delivery, annual bulk and reserve system gauge verification, and emergency medical-grade liquid bulk oxygen delivery and hookup. It aligns the price schedule with estimated annual bulk medical-grade liquid oxygen usage and requests a complete revised quotation using the revised price schedule. Attachment B – Clauses and Provisions and Attachment C – Statement of Work are also updated only to change CCF quantities, and the emergency medical-grade liquid bulk oxygen delivery service requirement remains.
