27-10-25 Addendum 2 (Contracted Physician Services)

Description: Houston Independent School District seeks statements of qualifications from qualified firms to provide contracted physician services addressing student medical needs, including services associated with Texas Health and Human Services Commission and Medicaid requirements. The selected physician will provide medical consultations and review clinician evaluations for health-related services such as speech, physical and occupational therapy, and audiology; the solicitation calls for a licensed physician with pediatric or developmental-pediatric experience. The contract is planned for December 1, 2026 through November 30, 2027, with two automatic annual renewals through no later than November 30, 2029, and a possible extension of up to 90 days.
