Putnam County Community Health Improvement Plan 2025-2030
The Putnam County Community Health Improvement Plan (CHIP) for 2025-2030 identifies three primary areas for strategic focus: Poverty, Anxiety and Stress, and Preventive Services. Developed in alignment with the New York State Department of Health's Prevention Agenda, the plan aims to address health disparities by targeting vulnerable populations, including older adults experiencing poverty, lower-income households facing economic instability and mental distress, and adolescents in specific zip codes with lower Human Papillomavirus (HPV) vaccination rates. Key strategies involve strengthening inter-agency collaboration, expanding Mental Health First Aid training, and implementing community-wide educational campaigns and provider-directed interventions to improve vaccination rates. The overarching goal is to enhance community health and well-being through evidence-based interventions and continuous evaluation.
