Crawford County Health & Human Services and Crossing Rivers Health Community Health Improvement Plan 2025-2027

The Community Health Improvement Plan (CHIP) addresses health needs identified in the Community Health Needs Assessment (CHNA) to improve the health and well-being of individuals in Crawford County and the Crossing Rivers Health service area. The plan is guided by three key priorities: enhancing mental health education, support, and services; improving chronic disease prevention and management; and strengthening substance misuse prevention and treatment. It outlines goals and strategies to address service gaps, barriers, and wellness needs through collaborative, sustainable efforts and resource alignment among local and regional partners, aiming for measurable progress in identified priority areas.
