This engagement will develop sustainable, outpatient ambulatory infusion services that are integrated with existing primary and specialty care (e.g., rheumatology, gastroenterology, neurology, infectious disease), serving eligible Central Health beneficiaries including uninsured, underinsured, and patients enrolled. To establish sustainable, outpatient clinic-based non-oncological infusion therapy services through a partnership between Central Health and CommUnityCare. This initiative will expand timely access to medically necessary treatments, improve care coordination across primary and specialty services, and reduce avoidable emergency department utilization for eligible patients. Initial contract duration: 1 year, contract renewal: optional - four (4) one-year renewal periods after initial term.
