Life and Accidental Death and Dismemberment Insurance (Request for Proposals)
The State Insurance Committee seeks to secure a single contract with an insurance company for the delivery of: 1. A group fully-insured, voluntary (employee pay all) term life insurance plan; 2. A group fully-insured, voluntary (employee pay all) Accidental Death and Dismemberment (AD&D) insurance plan; and 3. A group minimum premium insured basic term life and basic accidental death & dismemberment insurance plan (employer pay all). The benefit period for all plans shall be January 1, 2028, through December 31, 2032.
