With this funding, ACPHD seeks to implement a coordinated and sustainable system for HIV diagnosis, linkage or relinkage to HIV medical care, support for remaining engaged in HIV care, and achieving HIV viral suppression.

Purpose of the Emergency Housing Assistance Program is to provide short-term, housing-related support services to persons living with HIV in North and East regions of the County who are experiencing homelessness, housing instability, or imminent risk of housing loss.
