Case management services should involve assessment, planning, coordination, evaluation, and advocacy tailored to individual needs, emphasizing trauma-informed, customer-centered approaches that focus on supporting a successful move to maintain permanent housing. Case management should facilitate connections to mainstream and community-based services, including public benefits and/or entitlements; treatment resources for ongoing recovery and health needs, and other services such as job training, childcare, housing, skills development programs, credit repair and expungement services.
