Health-Related Social Needs (HRSN) Nutrition Support Services

Description: Pierce County Human Services Aging and Disability Resources is seeking qualifications from eligible organizations to provide Health-Related Social Needs nutrition support services to qualifying Medicaid recipients who are not currently receiving in-home long-term care support services. The services include Medically Tailored Meals and/or Fruit and Vegetable Provisions throughout Pierce County. This is an open and continuous solicitation for qualifications, not a grant program, and qualified organizations may apply at any time for the opportunity to negotiate a contract.
