Bridges to Wellness

Plan and execute a program of evidence-based, trauma-informed, school-based and community-based services to youth, and their families, attending the schools identified in the geographic area awarded to the contractor. The contractor must be able to provide these services to youth and families members with limited english proficiency and must be able to equally engage and serve families from multiple ethnic and racial backgrounds. Services must be ethnically and culturally relevant to the entire school population. At a minimum, the contractor's program must include behavioral health services, care management, and positive youth development services.
