Provide peer outreach and support services to individuals in approx. 22 medicaid-certified nursing facilities. To communicate and connect with nursing facility residents to inform them about their community living options and make referrals for options counseling, as needed. To support Nursing facility residents and empower them to transition successfully to the community with the right supports necessary to maintain independence.
