The Board determined Respondent substantially complied with all terms and conditions of the Consent Agreement and ordered his release.

The main issues discussed involved misrepresentations and omissions made by Czlapinski on his application for licensure, including failure to disclose previous disciplinary actions and criminal convictions. Ultimately, the Board voted to revoke Czlapinski's license.
