The coalition meeting focused on the State Health assessment and improvement process. Key discussion topics included an overview of the partnership, the importance of community engagement and collaboration, and the introduction of the vital conditions framework for promoting health and well-being. Participants discussed the goal of moving toward Upstream factors to address root causes of health disparities and reviewed the timeline and data collection efforts for the upcoming State Health Improvement Plan.

The meeting focused on the state health assessment and improvement plan. Key discussion topics included the review of the coalition's mission and vision, a refresher on the vital conditions for health and well-being framework, and an update on the state health assessment, which involved both primary door-to-door community survey data and secondary data sources. Additionally, the meeting covered community conversations held across various counties to gather feedback on resources and health barriers, a review of relevant policy scans, and an analysis of existing health disparities in Delaware, specifically regarding infant mortality, interpersonal violence, and drug overdose rates.

The meeting focused on providing an update on the State Health Assessment and State Health Improvement Plan, emphasizing an equity-centered approach. Key discussion topics included preliminary findings from community health surveys across all three counties, the importance of fostering trust, community engagement strategies, and an overview of demographic education data. Furthermore, the meeting involved a review of the coalition's strengths, weaknesses, opportunities, and challenges, and concluded with a recognition of student graduates and a discussion regarding the upcoming public comment period for the State Health Assessment report.

The summit focused on ecosystem mapping to address various health and social priorities, including housing, Medicaid expansion, healthcare access, transportation, and public health conditions like mental health and chronic diseases. Participants discussed current organizational efforts, identified gaps in community representation, and explored potential funding sources to improve vital community conditions and achieve more equitable health outcomes. Future actions were outlined, including an upcoming public comment period for the State Health Assessment, professional development opportunities through the Community Development Institute, and an equity awareness event.
