Perform nutrition & transit services combining three counties.
A. Nutrition Services: 1. Group Meal Program- A meal provided to a qualified individual in a group setting. 2. Home Delivered Meal Program- A meal provided to a qualified individual at a residence. 3. Meal Service-Prepared meals (hot, frozen and emergency meals) that are provided in the congregate and home delivered meal programs. 4. Evidence-Based and Health Promotions- Provide education and activities that support healthy lifestyles and promote healthy behaviors.
B. Transit Services: 1. Group Dining Transportation- Transportation provided to qualified individuals to and from the congregate dining meal site. 2. Social Group Transportation-Transportation provided to groups of qualified individuals to beneficial events and activities. 3. Essential Transportation- Transportation provided to multiple riders, starting from a single point of origin, going to a single drop. 4. Medical Transportation- Transportation services to qualified individuals to medical facilities, access to health resources, which promote health and independence.
