The purpose of the grant is to build stronger rural health networks, improve technology and shared operations, strengthen the rural health workforce and ultimately ensure that Vermonters receive the right care at the right time for an affordable cost.The purpose of this subrecipient grant opportunity is to provide funds to provider organizations for the purchase or lease of Remote Patient Monitoring (RPM) equipment and/or platform. RPM may be used in home health (in home and community settings), longitudinal care programs, long term care, primary care/specialty, and community paramedicine settings. RPM enables earlier interventions and ongoing monitoring for chronic conditions (e.g., congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), diabetes, hypertension, asthma for pediatric populations, etc.) in the home to prevent hospital admissions or readmissions. Examples of RPM equipment include, but are not limited to, internet/cellular connected tablets, smartphone applications, scales, blood pressure cuffs, pulse oximeters, glucose monitors, and cardiac monitors. Equipment is generally Bluetooth enabled and transmits data via wireless internet and/or cellular service, either through an intermediary device such as a tablet or smartphone application (including "bring your own device"), or directly to a vendor portal where clinical staff can review results and, where integrated, data flows into the provider’s electronic health record (EHR).
