Implement a department-level quality improvement program as necessary to meet the PHAB Domain 9 requirements for public health accreditation. This includes: Establishing a performance management system through promotion and utilization of VMSG, the SD DOH performance management system for progress monitoring, including: Monitoring performance management goals and the associated objectives with time-framed and measurable targets. Formalizing processes for VMSG utilization, including: Entering data in the performance management system. Monitoring data on performance. Communicating results on a regular reporting cycle. Using data to guide decision-making. Using data to facilitate continuous quality improvement (QI). Establishing linkages between VMSG and the SD DOH Strategic Plan. The anticipated award period is January 16, 2026-January 16, 2027, split between two contracts to align with the SD DOH contract period of June 1-May 31. The State will have the opportunity to renew the contract annually for up to five one-year time extensions. The extensions will not be automatic. See attached file.
