Provide oral health services that address oral health issues experienced by people with HIV in the Chicago Eligible Metropolitan Area (EMA). Initial contract period beginning March 1, 2026, through February 28, 2028, with up to two extensions, each not to exceed one year. See attached file.

Implement HIV primary care to promote the use of antiretroviral medications (ARV) for treatment of persons living with HIV in the Chicago Eligible Metropolitan Area (EMA) and HIV pre-exposure prophylaxis (PrEP) for persons vulnerable to HIV within the City of Chicago. Initial contract period of March 1 2026 to February 28,2028 with up to two extensions, each not to exceed one year. See attached file.
