Proposal must include completed scorecard Excel (MIN REQS and DEPT REQs) submitted in printed form and on two USB devices, active Shelby County EOC/Vendor numbers, signed EOC/LOSB forms (20% LOSB goal) and Drug-Free Workplace affidavit; award is a short-term contract with immediate start and renewal options.
