Provide resource coordination services for people with HIV, people vulnerable to HIV, and individuals impacted by syndemic infectious disease through the use of multiple funding sources, service categories, and modalities to support individuals identify needs and resources to address those needs, facilitate connections with resources including scheduling appointments, and following up with individuals to assess their satisfaction with the services they received. The initial contract period beginning March 1, 2026, through February 28, 2028, with up to two extensions, each not to exceed one year. See attached file.

Administer and manage all aspects of the expanded HIV screening. The Lead Organization will implement expanded healthcare setting-based HIV screening efforts for populations and geographies most impacted by disproportionately affected by HIV and syndemic infectious diseases. Initial contract period beginning March 1, 2026 through February 28,2028 with up to two extensions, each not to exceed one year. See attached file.

Syndemic Testing and Linkage to Care. Grant funds to outline the requirements and reporting expectations for organizations that agree to receive test kits and testing supplies to offer HIV, Hepatitis C, and Syphilis testing through ADHS in the service categories of Integrated Community Based HIV Testing, integrated Community Based Hepatitis C Testing and Integrated Clinical HIV Testing. Funds Available: $1,200,000.00. This Grant establishes an agreement between the Grantee and ADHS for the Grantee to receive in-kind support for Integrated Community Based Syndemic HIV, Hepatitis C, and Syphilis Testing and/or Integrated Clinical Syndemic HIV, Hepatitis C, and Syphilis Testing only in the form of HIV test kits, Hepatitis C test kits, and other supplies to conduct HIV, Hepatitis C, and Syphilis testing according to ADHS standards.
