Population Health Management Technical Assistance (RFI)

Gather information from entities regarding technical assistance (TA) to support a policy program under the Population Health Management Program (PHM). In January 2024, DHCS, in collaboration with the California Department of Public Health (CDPH), provided guidance in the PHM Policy Guide requiring Mange Care Plans MCPs participation in the Community Health Assessments (CHAs)/and Community Health Improvement Plans (CHIPs) conducted by Local Health Jurisdictions (LHJs) as outlined in All Plan Letter (APL) 23-021. The purpose of the TA is to ensure effective implementation of this policy by supporting and strengthening engagement among MCPs, LHJs and community stakeholders over time. Bridging gaps in understanding to align interpretations of policy expectations and enhance collaboration.
