Medical Case Management Ryan White Part A - (RFP)

Provide medical case management for persons living with HIV for eligible individuals with HIV in the Chicago Eligible Metropolitan Area. Services provided under this RFP are to be focused on individuals with HIV who are experiencing medical, behavioral, and/or social complexities which act as barriers to engaging in care and/or adhering to medical treatment for HIV. Examples include, but are not limited to, mental health concerns, substance use concerns, severe medical co-morbidities, and experiences of community violence, stigma, or isolation. Initial contract period beginning March 1, 2026, through February 28, 2028, with up to two extensions, each not to exceed one year.
