Providing behavioral health and support services for low income, homeless and/or precariously housed individuals. Service area includes: 1. Outreach, Prevention and Intervention Services to Address Domestic Violence and Resulting Trauma; 2. Promising and Evidence-based Prevention/Early Intervention for Children and Adolescents; 3. Services for a Teen Center; 4. Services for Youth Aging out of the Foster Care and/or Juvenile Justice Systems; 5. Public Health Initiative; 6. Intensive Case Management Services; 7. Supportive Housing and Case Management for Persons who have a Behavioral Health Diagnosis and are Experiencing Homelessness or are Precariously Housed; 8. Meals and Support Services for Persons Experiencing Homelessness; 9. Crisis Services to Children who have been Abused, Neglected, and Abandoned; 10. Food Distribution Services to Persons Experiencing Homelessness and/or of Low to Moderate Income; 11. Behavioral Health Services for Urban Native American Populations; 12. School-Based Substance Use Outpatient Treatment Services for Middle School-Age Youth; 13. Case Management for Persons who have a Behavioral Health Diagnosis and are Experiencing Homelessness or are Precariously Housed specific to the City/County Community Connections Supportive Housing Program.
Pierce County
Provide qualified organizations to deliver expanded access to wellness, prevention, and early intervention behavioral health services that are not reimbursable under medicaid or private insurance. These programs should provide upstream supports that help individuals sustain emotional well-being and prevent the onset or escalation of behavioral health challenges, especially among individuals, families, and other vulnerable or underserved populations. Services should promote resilience and whole-person care, reduce reliance on crisis and inpatient systems, and/or reduce disparities for those with behavioral health struggles. See outside link.
