Provides EAP services to approximately 190 fulltime employees. This quote should be presented in a "per employee" cost, with lump sum cost for the one-year period submitted on the form attached to this RFQ, and should include the cost of all services and printed materials as follows: 1) Assistance for employees in resolving issues including but not limited to substance abuse, mental health and emotional concerns, legal issues, relationship problems and stress management; 2) Provider shall provide Employee Assistance Program (EAP) services and up to six (6) counseling sessions per Township employee or EAP enrolled family member. These sessions should be for a 1-hour minimum. 3) On-site critical incident counseling and debriefing by an EAP Specialist as needed. This should include the availability of 24 hour/7 day per week crisis/emergency services; 4) Legal consultations and basic legal information from licensed attorneys to assist in the management of the employee’s personal life; 5) Quarterly newsletters regarding health, nutrition and overall well being; 6) Two (2) Employee Informational Workshops provided by medical and behavioral health professionals which address work/life topics; 7) One (1) training session for Supervisors and Managers to assist them in recognizing and assisting troubled employees and using the EAP program as an effective management tool; 8) An employee orientation for all Township employees will be held within 45 days of the award of contract, outlining the services available and how to access the EAP program; 9) Provide management reports on a quarterly and annualized basis. The reports should include aggregate EAP statistics reporting utilization rates for employees and their household members, demographic data, referral sources, presenting problems, contact types and any other pertinent information.
